What you’ll be doing
The purpose of this position is to provide a timely, safe and appropriate referral request into Child and Family and Complex and Chronic Community Health clinical services. The Intake Officer must collect and collate relevant demographic and clinical information to ascertain eligibility for service and using Microsoft Office and in house technology must correctly input the information to facilitate appropriate and sufficient detail informing the clinician who will receive the request for triage.
The Intake Officer is a front line position and the primary point of contact into Integrated & Community Health Services and provides service information across the care continuum to better inform the referrer and client decision making. The Intake Officer effectively communicates with patients, clients and other health care professionals in a respectful, courteous and positive manner and advocates the values of the Local Health District through ethical behaviour, professionalism and cultural diversity.

High level computer knowledge in data bases or Health information systems such as: iPM, CERNER, My Aged Care and Microsoft Word & Excel applications
Effective customer service and communication skills in verbal, written and interpersonal to develop and maintain relationships with client, team and clinicians
Excellent time management and organisation skills with attention to detail.
Demonstrated experience in referral pathways, person-centred care and patient privacy.
Demonstrated skill in the analysis of problem solving, implementing change and improvement strategies and monitoring outcomes.
